Adoption Choices of New York offers several service options for those seeking home study and post placement services. To begin receiving local adoption agency services, download the application, indicate what type of services you are looking for and mail back to Adoption Choices of New York.
NEW HOME STUDIES – After submission of the application and payment of the application fee, Adoption Choices of New York will review the application and forward you the Home study packet to complete and return to us. After all documents are submitted, a social worker will be assigned to schedule the home visit and interviews.
A HOME STUDY CONVERSION is required if you are seeking matching services through Adoption Choices of New York and you have an existing home study that was done by a private social worker or an agency other than Adoption Choices. After submission of the application and payment of the application fee, have your social worker or agency submit the completed home study and supporting documents and your attorney submit a copy of your Court Certification, if applicable. After Adoption Choices of New York reviews these documents, you will receive the home study conversion packet to complete and return to us. You will then receive a list of needed documents and forms, including child abuse and criminal clearance forms, as they are agency specific and non-transferable.
If seeking POST PLACEMENT SERVICES only, once the application and post placement packet has been completed and returned to us, we will notify ICPC that we are providing post placement services.

PRE-CERTIFICATION is a pre-adoption court process for adoption in New York State, which certifies (approves) applicants to adopt. The Pre-Certification process takes place in your local Family or Surrogates Court and includes an adoption attorney (of your choice) filing papers and Adoption Choices of New York submitting the home study, child abuse, and criminal clearances. Once granted, the Certification is good for 18 months.
LOCATING A BIRTHPARENT OR CHILD FOR ADOPTION
Once you have an approved home study and been pre-certified, you may begin your efforts to locate an expectant parent or child. All applicant(s) may work with outreach experts who may place ads, assist with on-line listings, and field phone calls from expectant parents. New York residents may NOT work with adoption facilitators. Applicant(s) working with Adoption Choices for a match/placement will work with our family marketing program to create you Adoption Profile Book, design a webpage and start your on-line campaign. Adoption Choices or adoption attorneys (for expectant parents located in other states) will help provide or coordinate services needed for a specific adoption.
HOME STUDY UPDATES For New York and most out of state placements, the adoption home study will need to be updated should you not have a child in your custody within 12 months. Medical reports and clearances are good for 12 months from the date received and may need to be updated prior to a home study update. Should a specific state need additional information, a home study addendum or update will be completed. Download the home study application.View List of Documents
